Is Palliative Care different to End of Life Care?

By definition, Palliative Care is the ongoing care of someone with a life-limiting or chronic illness to ease their pain and help to manage the symptoms and can last for several years.

We offer a holistic, compassionate approach for both Palliative Care and End of life care and work with you and your family to devise personalised care plans, ensuring they have flexibility built in to enable us to seamlessly adapt to your changing needs.

Our Carers are trained and skilled in knowing how to deliver this regardless of a Customer’s ability, diagnosis or prognosis.

Palliative Champion

The remit of our newly appointed Palliative Champion is to work with Customers and their families to complete advanced care planning in line with the Gold Standards Framework for Palliative Care. This provides assurance that the wishes, care needs and preferences of an individual are appropriately documented so that we can continue to provide person-centred care until they require end of life care.

End of life care at home

This is a very distressing time for all involved and our Carers will ensure your loved ones’ wishes are met whilst undertaking their duties with the utmost empathy and compassion. We will liaise with your GP and any other healthcare professionals that may be involved.

Moving to a hospice

Our Carers can help with arrangements to move your loved one to a hospice and will ensure the transition and process is managed with empathy for all involved.

Care after death

If you wish we can also provide care after death. Please speak to us to see how we can help in this situation.

Taking the next step

When you are ready to discuss things further please contact our office and we will arrange for an initial meeting with one of our senior Care Coordinators. This can include the person requiring care, any relevant professionals plus one or two other family members who may wish to attend. We can help to advise who should attend at the time of booking the appointment.

From this, we will gain a fuller understanding of the specific care required and can explain the different services we offer. A Care Plan will be created and all costs and payment options fully explained.
