We offer an extensive range of Domiciliary Care Services to support this and our Carers work closely with other health care professionals and social care teams to enable individuals to achieve this goal.
Our person-centred holistic care planning ensures everything we do is tailored to the specific needs of the individual regardless of their age, ability or general health and we will do our very best to match each customer with a Carer experienced with their particular needs.
Harmonize Care promotes integrated person-centred care planning to ensure our Customers receive seamless care across all health and social care domains. We are also fortunate to be able to reinforce the effectiveness of our delivery with an ongoing programme of training for all employees via our sister training company Harmonize Training harmonizetraining.com

Taking the next step
When you are ready to discuss things further please contact our office and we will arrange for an initial meeting with one of our senior Care Coordinators. This can include the person requiring domiciliary care, any relevant professionals plus one or two other family members who may wish to attend. We can advise who should attend at the time of booking the appointment.
From this we will gain a fuller understanding of the specific care required and can explain the different services we offer. A Care Plan will be created and all costs and payment options fully explained.
